Acknowledgment of Risk & Assumption of Responsibility

I, the undersigned, acknowledge and understand that participating in any Bayou Dog Sports Training Club event, including but not limited to training sessions, trials, evaluations, and other club-sponsored activities, involves inherent risks to myself, my dog(s), and any guests I bring. These risks may include, but are not limited to:

- Injury or illness to myself, my dog(s), or others from dog bites, scratches, or accidental contact.
- Property damage to personal belongings, equipment, or venue facilities.
- Exposure to environmental hazards, including extreme weather, wildlife, uneven terrain, and allergens.
- Unpredictable animal behavior, even in well-trained dogs, which could lead to injury.

I understand that for the safety of all participants and dogs, each dog attending the event must be up to date on vaccinations based on the American Veterinary Medical Association (AVMA) recommendations or have a valid titer test where applicable. I affirm that my dog(s) have received core vaccinations, including rabies, distemper, parvovirus, and adenovirus, as advised by my veterinarian. I assume full responsibility for any health-related risks associated with my dog’s participation.

I voluntarily assume full responsibility for any risks, injuries, or damages—foreseeable or unforeseeable—arising from my participation in Bayou Dog Sports Training Club activities.

Release of Liability

In consideration for being permitted to participate in club events, I agree to:

- Waive, release, and discharge Bayou Dog Sports Training Club, its officers, board members, event organizers, volunteers, sponsors, and any affiliated individuals or entities from any and all liability for personal injury, illness, property damage, or death arising from participation in club activities.
- Indemnify and hold harmless the club and its representatives from any claims, legal actions, or expenses (including attorney fees) resulting from my participation.
- Accept full responsibility for the behavior of my dog(s) and agree to take appropriate precautions to prevent harm to other participants, dogs, or event property.

Photo & Media Release:
I give permission for Bayou Dog Sports Training Club to use photographs, videos, or other media of me and/or my dog(s) taken at events for promotional or educational purposes.

Agreement & Signature:
